Streets to Home Program

Housing for 26 Persons who Had Been Homeless

The Streets to Home Program is supported by the Chicago Department of Family and Support Services. Their grant pays for most of the salaries of three case managers who provide general case management services to the homeless of Chicago. The case manager select 26 persons who are ready for housing. Each of these 26 persons receive a voucher for housing that allows them to get off the streets and end their homelessness.

Our case managers screen the participants, help them apply for housing, and help them get used to living indoors, if necessary. Our case managers continue to provide support so that the participants can stay housed. The case managers visit the participants in their new apartments and hold bi-weekly educational and social meetings with all participants. 

Since the program began in 2006, we have had 59 participants who have been placed into voucher-based housing. As of Dec 15, 2011...
43 remain housed, either in one of the 26 vouchered slots or in other housing in which they pay a portion of their rent
7 moved in with family or friends
2 are deceased
2 were incarcerated
2 are in a long term medical care facility
1 returned to the streets
2 whose whereabouts are unknown
